How to Add Custom Questions to Online Registration Form

ChildFriendly Pro allows you to tailor your registration process by adding specific questions that are in addition to the preset form. This feature ensures you collect the exact data needed for your center’s operations, from dietary needs to regional compliance requirements. By using a variety of formats like dropdowns and multiple choice, you can streamline the parent experience while maintaining organized, audit-ready records.

Step-by-Step Guide

1. Access Customizations

Navigate to Settings > Center > Customizations in your ChildFriendly Pro dashboard.

2. Enable the Feature

Scroll down to the Enrollment Questions section and toggle the feature to On.

3. Choose Your Question Format

Click to add a new question and select the format that best fits the information you need:

  • Text Field: Ideal for short answers or detailed long-form responses.

  • Number Field: Best for numerical data, such as emergency contact numbers or child ages.

  • Yes/No: A simple binary choice for quick confirmations (e.g., photo consent).

  • Multiple Choice: Allows parents to select more than one option from a list (e.g., specific allergies).

  • Single Choice: Restricts the parent to selecting only one option from a list.

  • Dropdown Menu: Provides a clean, compact list of options for parents to choose from.

4. Set Requirements

Select whether the question is Required (parents must answer to complete registration) or Optional.

5. Organize the Flow

Use the up and down arrows to reorder questions so the form feels intuitive for the families.

6. Save Your Progress

Click Save once you are finished to update your registration forms.

7. Manage Active Status

Ensure the Enabled toggle is on to make the question live. If you want to keep existing data but stop asking the question on new forms, simply toggle it Off to archive it.

Once a parent completes their registration, their answers to the custom questions will appear directly in their child's profile. You can find them in the dashboard under the Families tab. Room staff can also view these answers on their tablets by clicking into the child's profile from the attendance view.
